Care Pathways

Value Based Urgent Care
Value Based Urgent Care (VBUC) aims to reduce the number of people who present to a Western Sydney Emergency Department for urgent treatment of a condition that isn’t life-threatening by providing alternative Urgent Care Services that are free, local and patient-centred.
Cardiology in Community
Cardiology in Community (CIC) will strengthen participation and screening to improve the identification of people who are at risk of cardiovascular disease, in turn supporting ongoing management of heart failure, chest pain and atrial fibrillation in a general practice setting.
Mobile Imaging Services

Mobile Imaging Services (MIS) aims to reduce the number of patients presenting (or representing) to emergency departments or other hospital facilities by providing free in-home imaging services to patients across Western Sydney.

Urgent Care Service

Urgent Care Service (UCS) centres provide care for semi-urgent and non-life-threatening conditions. An Urgent Care Service centre is a general practice that has been upskilled and resourced to treat patients with medical conditions that require urgent treatment. UCS centres aim to take the pressure off of hospital emergency departments by providing patients with community-based care for acute illnesses or injuries that would normally be triaged as a category 4 or 5.

About Us

Western Sydney Care Collective is a partnership between the Western Sydney Primary Health Network and the Western Sydney Local Health District. Through the Value Based Urgent Care and Cardiology in Community pathways, Western Sydney Care Collective aims to deliver the vision of ‘one Western Sydney Health System’. Service gaps are collaboratively commissioned through the Western Sydney Primary Health Network’s strategic commissioning function and Western Sydney Local Health District resources are realigned to support the care pathway design.

Through integrated governance, delegations, shared culture, information sharing, community engagement and communications, Western Sydney Care Collective allows the Western Sydney Primary Health Network and Western Sydney Local Health District to overcome organisational barriers and work together to support the patients, families and carers who need care.
